Description

Most of the templates I found online for medical ID cards were very sparse and didn't have nearly enough space. I needed something that had more room but still fit in my wallet. To that end, I've made an accordion-fold style medical ID Card template. There are 3 sizes to choose from. If your going to use either the regular or extended template I would recommend plain paper or lightweight card stock at most, since it can be hard to fold with regular card stock. After filling in your information and printing it out, simply cut along the dotted lines and then fold it on the solid lines alternating the fold direction.
