Font used, but not critical: Bookman Old Style. There a subsitute field for the picture of the cover page. It uses four styles of pages : First page, Index and left and right pages. The how to is in the template itself. And, yes, feel free to translate it. The screenshots show how the pattern comme, and how it is actually used in small book.
